Appointment Checklist 

1) Four hours before your appointment start time you will recieve a COVID-19 questionnaire to your e-mail you signed up with. Please fill this out. 

If you arrive to the clinic and show ANY symptoms of illness, I will immediately stop the treatment. Even though the situation has improved locally, I still have a no symptom tolerance policy.

2) The waiting room is currently closed, upon arrival you may either text or call (204)-724-4472 and let me know you are ready to be seen.

3) Masks are optional for your time inside the clinic, in your COVID-19 survey you can answer whether or not you will want your therapist to wear a mask. If I deem the situation to require a mask, I reserve the right to change this policy at any time.

4) Sanitizer is still available in the clinic and I encourage the use of it before and after treatment.

Every appointment will have an extended 30 minute break in between to allow me to properly sterilize the environment.

PERSONAL PROTECTIVE EQUIPMENT

If you require a mask and don't have one available, I will provide one.
